Safe Transition Council appoints Susan Fitzgerald from Hickory House in Honey Brook

Susan Fitzgerald, a physical therapist with Hickory House Nursing and Rehabilitation Center in Honey Brook, has been appointed a member of the Safe Transition Council for Life Care Centers of America.

Life Care Centers of America (LCCA) is a national company with 250 skilled nursing facilities throughout the United States with a strong mission to meet the needs of the aging population. Each beautiful, well equipped facility is staffed with caring competent professionals. Hickory House in Honey Brook is the only Life Care Centers of America facility located in Pennsylvania.

Committed to providing excellent providing excellent, effective care in the changing world of healthcare, the leadership of LCCA selected fifteen healthcare professionals from facilities across the country to form the Safe Transition Council. The council’s goal is to develop evidence based programs, processes and procedures to ensure that all LCCA patients experience both effective healthcare and a seamless transition as they move from either their home or a hospital, to a Life Care facility, and back to the community. Creating effective and seamless transitions improves the quality of care patients receive, decreases re-admissions to the hospital, and enables patients to return to their lives in their homes and community.

Over the next two years, Susan will be working with case managers, other therapists, nurses, liaisons and management staff to focus on keeping LCCA and Hickory House at the forefront of quality patient care and successful outcomes. Through the council, both leaders at the corporate level to associates from facilities such as Hickory House will work together and dedicate themselves to their mission while providing the best possible subacute and long term care available.
